What this letter is
An insurance appeal letter formally asks an insurer to reconsider a denied or underpaid claim. Insurers run appeals as a written process with a deadline stated in the denial letter — often 30 to 180 days from the date of the denial, depending on the plan and the type of coverage. Missing the window usually ends the appeal, so the date you send it matters as much as what it says.
What to put in it
- Your name, policy or member number, and the claim number from the denial.
- The date of the denial letter and the specific reason the insurer gave.
- The service, treatment or loss being claimed, with dates.
- Why you believe the denial is wrong — factual, not argumentative.
- Supporting documents: the denial letter, bills, records, or a provider's letter.
- A clear request that the claim be reconsidered and paid.
Appeal deadlines are counted in days, and the burden of showing you filed on time falls on you. Certified Mail gives you tracking and a record of delivery against the date printed on the denial letter.

Good to know.
How long do I have to appeal?
The deadline is set by your plan or policy and is stated in the denial letter itself — commonly 30 to 180 days from the denial. Read the denial for your own deadline and allow mailing time inside it; we typically hand a letter off within 1–2 business days and First-Class delivery usually takes about 3–5 more.
Can I include the denial letter and my medical bills?
Yes — upload them and they are printed and mailed behind your letter. Genie can also read the dates, claim numbers and amounts out of them and use those in the draft. Letters run up to 12 pages.
Is my health information kept private?
Your letter and any uploads are encrypted in transit and at rest and stored in the United States. Drafting runs through a US-based AI provider that does not train on your content and retains it only briefly. We never sell your data.
